One truck safety organization is conducting several events this summer that highlight and attempt to prevent common causes of catastrophic trucking accidents.

The Commercial Vehicle Safety Alliance is a coalition of roadway safety groups and law enforcement throughout North America.  It focuses on making trucking safer in the United States, Canada and Mexico.

On June 27, an Amtrak train headed for Chicago from Los Angeles hit a dump truck at a Mendon, Missouri, crossing and derailed.  At least three people – two train passengers and the dump truck driver - were killed.  At least 50 people were injured as a result of the derailment, which occurred in rural Missouri, about two hours north of Kansas City, Missouri.

There were 243 passengers and 12 crew workers onboard.

The National Transportation Safety Board is sending a team to investigate the deadly train crash. Reportedly eight train cars derailed as a result of the collision.

A hospital-acquired infection is a potentially deadly medical error.  They largely are preventable, and one hospital recently provided a blueprint for preventing one of the most serious hospital-acquired infections.

These occur when bacteria enter a patient’s body during a hospital stay.  They typically are caused by germs spread by medical staff or unclean medical devices.

Preventing fatal commercial truck crashes continues to be a national concern, and lawmakers are targeting speeding truck drivers.

The most recent federal data show that in 2020, nearly 5,000 people were killed in commercial truck accidents.  More than 70% of those who died were occupants of vehicles other than the truck.

Catastrophic crashes involving tractor-trailers and other large commercial vehicles appear to be growing problem. The federal government estimates that fatal commercial truck crashes jumped 13% in 2021 over 2020.

In 2017, a Tennessee hospital nurse gave a patient the wrong medicine, administering a strong paralytic drug rather than an anti-anxiety drug. The patient died as a result.  A patient safety organization recently announced that such medication errors are avoidable and detailed how all hospitals can prevent them.

The Anesthesia Patient Safety Foundation released a safety advisory following the organization’s review of the Tennessee hospital fatal medication error that has received widespread media attention.

According to the most recent federal report of fatal crashes, the number of people killed in motor vehicle accidents last year was the highest it has been in 16 years – including a 13% jump in commercial truck crash deaths.

In April the National Highway Traffic Safety Administration released a report that estimates the number of U.S. car and truck crash deaths for 2021. NHTSA projects that nearly 43,000 people died in catastrophic motor vehicle accidents.  If the projection holds after the numbers are finalized, that would represent a 10.5% increase over 2020.

A 2016 study concluded that as many as 250,000 people in the United States die every year from a medical error.  A new study finds that one out of four Medicare hospital patients is a victim of a medical error, with nearly half of the errors being preventable.

A Johns Hopkins University study released in May 2016 (“Medical Error—The Third Leading Cause of Death in the US”)  determined that medical errors at the time were America’s third leading cause of death, behind only heart disease and cancer.
